Led by founders Lisa Kolton and Kim Hamilton, our team of social workers and care managers brings years of hospital, rehab, mental health and dementia experience to every family we help.

When my 101 year old mom suddenly had a stroke, her needs changed instantly. Thanks to Lisa, I was able to handle the caring and grieving without being distracted by the business of health care. We will be forever grateful.
Kim was indispensable to my family in all aspects of senior care. I would never have been able to maintain my sanity without her during multiple serious medical events involving my parents.
When my elderly patients leave my office with Lisa, I know they are in good hands. Lisa is exceptionally caring and kind and truly dedicated to her clients.
When my family faced nursing home decisions for my mother, Lisa rescued us from a wrong decision. Knowing the system requires a true professional, and Lisa is the professional to have.

We come to you: at home, in the hospital, in rehab or in a care community. Choose your county to find your town.
No. We work with older adults, adults living with mental illness, and people with developmental or other disabilities. With every client we work to preserve dignity and self-determination by involving the client and family throughout.
We start with a family meeting or call to learn the medical history, social history and your main concerns. Then we schedule a time to meet your loved one, with or without family. It usually feels like a friendly visit. Afterwards we explain the options, write a care plan and make specific recommendations based on our direct experience with local services.
Background and health history, ability to manage daily life in the current home, safety concerns, finances, family and informal support, and any formal services already in place. We also look at the home itself to see whether changes could help.
Typically 3 to 5 hours, though it can vary with the situation.
Medicare and standard health insurance do not cover private care management. Many long term care insurance policies do, so check yours or ask us to help you review it.
Some families take our recommendations and put the plan in place themselves. Others, especially those who live far away or work long hours, keep us on for ongoing support: making sure services are working, going to doctor visits, staying with clients in the ER, fighting insurance denials and keeping the family updated.
The first conversation is free and there's no obligation. Call either of us directly, or send a note and we'll get back to you within one business day.
